A practical guide

  1. Choose the PDF to make fillable.
  2. List one field per line: name | type | page | x | y | width | height, in percent of the page from the top left.
  3. Check placement, create the form, then fill every field in a PDF viewer.

One PDF. Up to 100 fields: text, multi-line or checkbox; names up to 40 characters (letters, digits, _ . -); positions in percent of the visible page.
